The provider was missing 1 employment application for the new employee. Technical assistance was provided regarding the importance of having all necessarily documents ( Background screening & personnel file requirements application ) in the Employee file for review and enter the new personal in the system (CARE). Per the CCF Handbook, section 44.2 Child care personnel did not have an employment application on file. CCF Handbook, Section 7.4, A Child care director acknowledged and printed the form and place it in the employee file.

Inspector notes
FSC observed bedding in multiple rooms to be unsanitary. A set of bedding was observed in bathroom of facility, another set was observed to be stacked onto of each other with linen already on cots. Technical assistance was given to provider per CCF handbook " G. Bedding and linens, when not in use, must be stored in a sanitary manner which prevents the spread of germs or lice from other linens. All bedding and linens must be thoroughly cleaned and sanitized at least once a week, and before use by another child. H. Bedding and linens shall not be stored in the bathroom, unless stored in cabinets. "
