Upon reviewing the roster in the clearinghouse, licensing specialist determined that the employer failed to remove 1 staff from the roster within the required timeframe after employment termination. TA: According to section 435.12(2) (c) an employer of persons subject to screening by a specified agency must register with the clearinghouse and maintain the employment status of all employees within the clearinghouse. Initial employment status and any changes in status must be reported within the required timeframe.

The parent/guardian of a child who has not received the age-appropriate immunizations prior to enrollment and who does not have documented medical or religious exemptions from routine childhood immunizations must provide documentation of a scheduled appointment or arrangement to receive immunizations.
