At time of inspection it was witnessed that a staff member was not hired on original date of 9/6/22 documented on the 5131, instead staff started on 1/2/23 which was a 90-day break in service. Assistant Director requested a re-submission in the Clearinghouse putting the facility back in compliance for this standard.
