There were two students that did not have a current Immunization record. The childcare facility is responsible for obtaining for each child in care, a current, complete and properly executed Florida Certification of Immunization form Part A-1, B, or C, DH 680 form or a Religious Exemption from DH 681 form. All 22 Student Immunization Records were reviewed.
