The guardian was contacted, but a Incident/Accident report of the incident was not completed and signed off on by the guardian on the day of the incident. The standard was brought back into compliance by the facility completing and Incident/Accident report on 6/14/2024. The facility was not able to obtain a signature as the child no longer attends. Counselor reviewed vehicle insurance for all four vehicles which is valid through 8/31/2024. Counselor reviewed the vehicle safety alarm for the vehicles used to transport children. Part of the alarm mechanisms were not in proper working order and in accordance with the alarm requirements.
