One staff member started industry 1/27/23 at another facility. Left that facility and started working at this facility on 7/24/24. Transcript shows that this staff member signed up for DCF training on 3/23/23 and did not complete required training for DCF Part I and Part II as required before being employed at this facility. 4.3 Break in Service page 43 B.
