- On March 20, 2025, a child snuck away from a teacher as the teacher was retrieving another child and was left on the playground hidden from view. The child knocked on a window of another classroom and the teacher escorted him to his classroom. One of the teachers in the 2-year-old classroom was terminated on 3/21/2025. The Child Care Facility handbook states on page 12: 2.4.1 General Supervision Requirements A. Child care personnel must position themselves in the outdoor play area so that all children can be observed and directly supervised.

- Specialist interviewed the teacher that witnessed the incident February 20th, 2025. She was busy and forgot to notify the parent or have the parent sign the accident/incident report when the child was picked up. When the parent came to pick the child on the last day of enrollment, they asked for all accident/incident reports involving their child.
