- Child care personnel failed to share the accident/incident form with the custodial parent/ legal guardian or the individual authorized to pick up child on the day that the incident occurred. CCF Handbook, Section 6.4, B & D
- Report comments
- During the onsite inspection, it was found that an incident/accident that occurred on 1/28/2025, was not given or signed by the parent on the date of occurrence. During a review of the last three months of incident/accident reports, another incident/accident report was found that showed the report was not signed on the day of occurrence. The facility will continue to use the methods currently in place to have parent's sign the reports but will be more diligent in ensuring the reports are signed on the day of occurrence.
