Children in the outdoor play area were not adequately supervised in that [a teacher could not supervise a group playing while she was sitting down under the structure.].

A teacher was sitting under the playground structure, doing work on her phone, while the children were playing. A child was pushed off of the equipment and broke his arm. The teacher was unable to see the situation unfold, nor was she able to see the fall, due to her location.
