- At time of inspection, Counselor observed playground gate leading to the parking lot to be open even though there was a lock in place. Counselor closed the gate leading to the parking lot and it still pushes open while being locked. Technical assistance provided that the gate must be secure at all times, so children do not exit the outdoor play area.

A child on the playground cut their hand on the sharp jagged edges while going down the slide. Technical assistance provided ensuring that all play equipment is safe and sanitary. The provider removed the play equipment with rust and jagged edges and the one that the child harmed themselves on.
