During the inspection licensing reviewed 13 new children's files and out of the 13 four of them had expired shot records. Please see confidential page. Technical Assistance given from the handbook: 7.1 Immunization Records: It is the provider's responsibility to make sure all immunization records are up to date unless custodial parent provides a religious exemption form. Corrective Action: Provider must email licensing the required shot records on or before 12/19/2025
