Inspector notes
Upon arrival to the facility for a routine inspection, Licensing Specialist observed the facility flooded with water damage. Licensing Specialist observed the wet ceiling tiles in entry ways, hallways and classrooms. Provider advised Licensing Specialist that she arrived yesterday and witnessed an extreme amount of water coming from under the front door of the facility. Provider called maintenance and they stated that the water was coming from an apartment above the facility. Provider stated that the leak was supposedly repaired and tiles were replaced. Provider stated that upon arrival this morning, she was met with water coming from the front again. Provider stated that she ... [truncated]
