Inspector notes
LS reviewed all three personnel records. The employee listed on the Supplemental sheet had not signed this form since 5/16/25. She signed one today. Director stated, she will have all staff sign this form in the future when she signs it at license renewal time to prevent this from occurring in the future. Technical support provided. C. CF-FSP Form 5337, Child Abuse & Neglect Reporting Requirements, which is incorporated by reference in 65C-22.001(7)(l), F.A.C., must be signed on or before hire date and annually thereafter by all child care personnel.
