- The facility did not have documented proof that all child care personnel were trained and knowledgeable within 30 days of date of hire in [ safe sleep practices and shaken baby syndrome]. CCF Handbook, Section 4.2.3 and 4.2.5 Training [SR]

- Licensing Specialist reviewed personnel training records and observed one training record that showed safe sleep and shaken baby syndrome requirements had not been completed. See supplemental. Corrected prior to uploading. Technical assistance was provided regarding completion of safe sleep practices and shaken baby syndrome training requirements. Licensing Specialist received a electronic message from the personnel on 05/22/2026 with certificate of completion.
