When one-year-olds were transitioning back inside the building at the end of their outside time, one child was accidentally left behind. A teacher on an adjacent playground noticed the unsupervised child and took him to his classroom. The classroom teacher was counseled on transitioning procedures and written up by management.

During the complaint investigation, the outdoor play area for the one-year-olds had the following hazard: A hole dug behind one of the pieces of equipment, which exposes a large rock; the exposed rock poses a potential for harm to the children. Provider will remove the rock, fill the hole and email a picture of the completed work to the licensing office on or before 6/25/2026. Suggestion: An addition of a sandbox may prevent the children from digging in other areas of the playground.
