- An abnormality to the child's head was first noticed by staff at approximately 6:10 am. Staff observed an area of the child's head swell up throughout the morning. According to parents they were not notified until they received a phone call at 12:10 pm asking them to pick the child up. Staff's inability to identify a serious symptom of head trauma led to a delay in notifying the parents. On August 11, 2025, CPT medical staff subsequently diagnosed a healing skull fracture.
