This requirement was not met as evidenced by: CCLD was made aware of the Unusual Incident on 11/14/25, when the incident occurred 11/6/25, which is outside of the 24 hour reporting requirement, which poses a potential health, safety or personal rights risks to children in care
(a) The licensee shall provide care and supervision... (1) No child(ren) shall be left without the supervision of a teacher at any time...Supervision shall include visual observation. This requirement was not met as evidenced by: Based on interviews, the facility did not comply with the section cited above where one child was left without 100% supervision, which poses an immediate health, safety or personal rights risk to persons in care.
