A teacher shall supervise no more than 14 children or with an aide a maximum of 28 children.... this regulation was not met.. LPA conducted interviews and reviewed documents that determined... on 12/19/24, from 1:00 PM to 1:21 PM, the school-age program had 12 children attending, but it is unclear which staff member(s) provided care and supervision. Also on 12/20/24, from 1:08PM to 1:21PM, Staff #17(aide) was the only staff documented to be at the facility providing care and supervision to 12 children.
