Based on LPA's observations, documents collected and interviews,the facility was found to be out of complaince with the aforementioned citation... On 12/06/2024, from 1:11PM to 1:48PM it... is unclear who supervised 15 infants. On 10/29/2024, from 11AM to 11:45AM It is unclear who supervised 16 infants. On 10/24/2024 it is unclear who supervised the 17 infants. On 9/23/2024 from 12PM to 1PM there are 17 infants in attendance, it is unclear who supervised the 17 infants.
