This requirement is not met as evidenced by: Based on record review, the licensee did not comply with the section cited above. Today, LPA Ocegueda conducted a file review of 6 children's files and observed that child #3, #4, #5, #6 were missing updated immunization records. This poses a potential health, safety or personal rights risk to persons in care.

If no assistant provider is present at a Large Family Child Care Home, then the licensee shall comply with the capacity requirements for a Small Family Child Care Home as specified in subsections (b) and (c). This requirement was not met as evidenced by record review and interviews. Licensee stated that on approximately two occassions in early September 2022, she cared for infants #1,#2,#3 and #4 as well as child #5 alone for approximately 20 minutes causing her to be out of ratio. This poses a potential health, safety and or personal rights risks to persons in care.

This requirement is not met as evidenced by: Based on observation the licensee did not comply with the section cited above. Licensee sits next to the infants while they sleep in cots on the ground. A record review indicates that infant #1 and infant #2 were recently enrolled and licensee stated she was aware she needed a crib and she is in the process of obtaining a crib. This poses a potential health, safety or personal rights risk to persons in care.
