REPORTING REQUIREMENTS- Upon the occurrence, during the operation of the child care center of any of the events specified in a report shall be made to the Dept. by telephone or fax within the Dept. next working day & during its normal business hours. In addition, a written report containing the information specified shall be submitted to the Dept. within 7 days following the occurrence of such event. Any injury to any child that requires medical treatment. This requirement was not met as evidenced by staff interview. Staff did not report an Unusual Incident in a timely manner. This poses a potential risk to the health, safety,or personals rights to children in care.
