Limitations on Capacity: A licensee shall not operate a child care center beyond the conditions and limitations specified on the license, including the capacity limitation. This requirement is not met as evidenced by: Based on observation and record review, the licensee did not comply with the section cited above. Since August 2025, the Director allowed children in care to use a room that was previously deemed inoperable due to a flood. The room change was not reported to CCL and classroom was not inspected by the Department prior to children's use which poses an immediate health, safety or personal rights risk to persons in care.

Sign In and Sign Out (b) The person who brings the child to, and removes the child from, the center shall sign the child in/out. This requirement is not met as evidenced by: Based on observation and record review, the licensee did not comply with the section cited above. During today's inspection, LPA observed Child #1 (C1) was removed from the center by Adult #1 (A1) before naptime. Records review of the sign in/ sign out sheet showed that C1 was not signed out by A1 which poses a potential health, safety or personal rights risk to persons in care.

Such structural changes shall include, but not be limited to, room additions. This requirement is not met as evidenced by: Based on observation, interview, and record review, the licensee did not comply with the section cited above. The Director did not report a flood that took place in the licensed preschool class room in December 2024. The flood resulted in a room change that was also not reported to the Department which poses a potential health, safety or personal rights risk to persons in care.
