Outdoor Activity Space (d) The surface of the outdoor activity space shall be maintained: This requirement is not met as evidenced by: Based on observation, the licensee did not comply with the section cited above. LPA observed multiple tripping hazards in the outdoor play area. There are tree roots that are uncovered and ground is uneven. Sidewalk is lifted and there are mutliple cracks on sidewalk, which poses a potential health, safety or personal rights risk to persons in care.

Report finding
Responsibility for providing care and supervision: No child(ren) shall be left without the supervision of a teacher at any time, except as specified in Sections 101216.2(e)(1) and 101230(c)(1). Supervision shall include visual observation. This requirement was not met as evidence by child#1 was left outside and unaccounted for approximately 10 minutes. As a result the child was observed crying when staff discovered he was unaccounted for. This is an immediate health and safetyy risk.

Administration of Child Day Care Licensing (1) On or before March 30, 2018, a person who, on January 1, 2018, is a licensed child day care provider, administrator, or employee of a licensed child day care facility shall complete the mandated reporter training provided pursuant to paragraphs (2) and (3) of subdivision (a), and shall complete renewal mandated reporter training every two years following the date on which he or she completed the initial mandated reporter training. This requirement is not met as evidenced by: Based record review the licensee did not comply with the section cited above. Upon inspection of 4 staff files, it was observed that staff either had expired Mandated Reporter Training or not available in the file for review. This poses/posed a potential health, safety or personal rights risk to persons in care.
