Infant Safe Sleep Documentation shall be maintained in the infant’s file and be available to the Department for review. Documentation shall include the following: Time of each 15-minute check This requirement is not met as evidenced by: During today's inspection, Licensee was not able to produce 15-minute sleep logs for the enrolled infants for the month of December 2023. LPA observed sleep logs for the months prior. Licensee stated there were no infants in care during December, however Licensee was unable to provide proof. Based on observation and record review, the licensee did not comply with the section cited above, which poses/posed a potential health, safety or personal rights risk to persons in care.

Staffing Ratio and Capacity (d) For a Large Family Child Care Home, the maximum number of children for whom care may be provided at any one time when there is an assistant provider in the home, including children under age 10 who reside at the licensee's home and the assistant provider's children under age 10, shall be either: (1) Twelve children, no more than four of whom may be infants; or This requirement is not met as evidenced by: During today's inspection, LPA took a census of the children in care, and observed the Licensee and two assistants to be caring for a total of eleven children, including six that are infants. The licensee was observed to be operating out of ratio. Based on observation and record review, the licensee did not comply with the section cited above in, which poses a potential health, safety or personal rights risk to persons in care.

Personal Rights (a) Each child receiving services from a family child care home shall have certain rights that shall not be waived or abridged by the licensee regardless of consent or authorization from the child's authorized representative. These rights include, but are not limited to, the following: (1) To be treated with dignity in his/her personal relationship with staff and other persons. During the inspection it was revealed that Staff #1 and Licensee disclosed confidential information about a day care child to someone who is not related or does not attend the day care home. This poses a potential risk to the health, safety, or personal rights of children in care.
