This requirement was not met as evidenced by: Based on observations, record reviews, and interviews, the licensee provided care for five infants and 10 preschool-aged children on 03/19/2025, and three infants (up to 24 months) and 10 preschool-aged children (ages 2 to 5) on 04/03/2025. The licensee was out of compliance with both ratio and capacity requirements on both dates. This poses an immediate risk to the health, safety, or personal rights of children in care.

These rights include, but are not limited to, the following: (1) To be treated with dignity in his/her personal relationship with staff and other persons. Based on LPA interviews and informations received, a child’s personal rights were violated. Licensee did not comply with the section cited above which poses an immediate health, safety, or personal rights risk to persons in care.

When circumstances require the licensee to be temporarily absent from the home, the licensee shall arrange for a substitute adult to care for and supervise the children during his/her absence. Temporary absences shall not exceed 20 percent of the hours that the facility is providing care per day. Based on LPA interviews and information received, Licensee did not provide adequate supervision for children in care.
