101223 Personal Rights (a) The licensee shall ensure that each child is accorded the following personal rights: (1) To be accorded dignity in his/her personal relationships with staff and other persons. This requirement was not met as evidenced by: Based on record review and LPA observation of surveillance cameras, Staff #1 removed the infant from the chair, and abruptly pulled the bib off from the infant's neck upward over the child's head, startling the infant and causing the infant to wake up and cry. This poses a potential risk to the personal rights of children in care.

This requirement was not met as evidenced by: Based on interviews and LPA observation, on 05/30/2025, Staff 7 was observed spanking Child 3 once on the buttocks, lifting the child, turning the child around and sitting the child down. In addition to spanking a child, the same staff was also observed inappropriately handling Child 4 in care by dragging the child by the feet and picking up Child 4 from the ground by their wrists/forearm.
