All individuals subject to a criminal record review pursuant to Health and Safety Code Section 1596.871 shall prior to working, residing or volunteering in a licensed facility: (1) Obtain a California clearance or a criminal record exemption as required by the Department This requirement was not met as evidenced by: Based on interview and record review, S1 was working with children at the facility without an associated criminal record clearance, which poses an immediate health & safety and personal rights risk to the children in care.

Supervision shall include visual observation. This requirement was not met as evidenced by: Based on interviews conducted by the LPA and the center director’s own admission, it was corroborated that on 04/02/26 at approximately 09:30am, child C1 was without supervision of a teacher for approximately two minutes while in the classroom before being reunited with their class. This posed a potential health, safety or personal rights risk to persons in care.
