Report finding: The licensee shall provide care and supervision as necessary to meet the children's needs.(1) No child(ren) shall be left without the supervision of a teacher at any time, except as specified in Sections 101216.2(e)(1) and 101230(c)(1). Supervision shall include visual observation. This requirement is not met as evidenced by: Based on the Licensee’s own admission on 09/04/2025 child C1 was left without staff supervision on the play yard by the garden gate. This posed an immediate health and safety risk to the child(ren) in care.

Report finding: No child(ren) shall be left without the supervision of a teacher at any time, except as specified in Sections 101216.2(e)(1) and 101230(c)(1). Supervision shall include visual observation. This requirement was not met as evidenced by: Based on interviews with current and former staff conducted between 05/02/23 and 07/25/23, it has been determined that facility staff failed to provide adequate care and supervision to a group of children playing both indoors and outdoors on more than one occasion when a group was let inside or outside without a teacher present, resulting in a lack of supervision which poses a potential health, safety or personal rights risk to persons in care.

This requirement was not met as evidenced by: Based on interviews conducted between 05/02/23 and 07/25/23 there were uncovered electrical outlets located in two locations in the middle classroom on more than one occasion, which poses a potential health, safety or personal rights risk to persons in care.
