Infant Safe Sleep Documentation shall be maintained in the infant’s file and be available to the Department for review. Documentation shall include the following: This requirement is not met as evidenced by: Based on interview, the licensee did not comply with the section cited above. At 2:10pm L1 stated that she is not completing sleep logs for the two infants (C6 & C7) in care, documenting the infants Name, Date, and Time on each 15 minutes sleep check which poses a potential health, safety or personal rights risk to persons in care.
