Modifications to Infant Needs and Services Plan (a) The written infant needs and services plan shall be updated at least quarterly, or as often as necessary to assure its accuracy. This requirement is not met as evidenced by: Based on seven children's (C1-C7) records reviewed at 1:48pm which revealed C1, C3-C4, & C6-C7's Infant Needs and Services Plans had not been updated quarterly. The licensee did not comply with the section cited above which poses/posed a potential health, safety or personal rights risk to persons in care.

Infant Care Center Sleeping Equipment (b) A crib or portable -crib meeting United States Consumer Product Safety Commission safety standards shall be provided for each infant who is unable to climb out of a crib. This requirement is not met as evidenced by: Deficient Practice Statement Based on LPAs' observation of infants napping on cots and the Center Director did not furnish evidence to prove the children were able to climb out of the crib.
