This requirement is not met as evidenced by: Based on children's records reviewed at 10:51am and Licensee not furnishing proof that 15 minutes checks had were being conducted for C5 & C6. This requirement is not met as evidenced by: Based on children's records reviewed at 10:51am which revealed C1-C6's records either did not contain Immunization Record (IR) that were transcribed onto the blue CDPH 286 or transcribed CDPH 286 were missing.
