Operation of A Family Child Care Home (3) A file of affidavits signed by each parent with a child enrolled in the home. The affidavit shall state that the parent has been informed that the family child care home does not carry liability insurance or a bond according to standards established by the state. This requirement is not met as evidenced by: Based on record review Licensee reported she does not carry liability insurance for the daycare and she is not required to. LPA agreed with licensee and reminded licensee although it's not required to obtain liability insurance, you are required to have LIC 282 signed and on file for each child's record. The licensee did not comply with the section cited above which poses a potential health, safety or personal rights risk to persons in care.

Licensee reported she usually has an assistant present, how-ever assistant was on vacation currently and she believed she could have eight children present if they were under five years old. LPA printed out regulation and reviewed it with licensee. LPA also gave licensee a ratio hand out illustrating the regulation. the licensee did not comply with the section cited above which poses a potential health, safety or personal rights risk to persons in care.
