Administration of Child Day Care Licensing (1) On or before March 30, 2018, a person who, on January 1, 2018, is a licensed child day care provider, administrator, or employee of a licensed child day care facility shall complete the mandated reporter training provided pursuant to paragraphs (2) and (3) of subdivision (a), and shall complete renewal mandated reporter training every two years following the date on which he or she completed the initial mandated reporter training. This requirement is not met as evidenced by: Based on record review, the licensee did not comply with the section cited above, Staff S4 does not have a current mandated reporter training which poses/posed a potential health, safety or personal rights risk to persons in care.
To be free from corporal or unusual punishment, infliction of pain, humiliation, intimidation, ridicule, coercion, threat, mental abuse or other actions of a punitive nature including but not limited to: interference with functions of daily living including eating, sleeping or toileting; or withholding of shelter, clothing, medication or aids to physical functioning.
Cribs shall be free from all loose articles and objects, including blankets and pillows. Based on LPA's observation, one infant was allowed to sleep in a crib with a bottle. This is a potential health and safety risk to children.

Health and Safety Code Section 1596.8595 (c) A licensed child care facility or home shall provide to the parents of each child receiving services in the facility copies of any licensing report that documents any Type A citation that represents an immediate risk to the health, safety, or personal rights of children in care as specified in paragraph (1) of subdivision (a) of Section 1596.893b. This requirement is not met as evidenced by: missing documentation for C1, C2, C3, C11, C12, C13 & C14 as based on file review and interviews the facility failed to provide copies of type A violations to Children's representatives. Which poses/posed a potential Health and Safety risk to children in care.
An aide assisting a fully qualified teacher shall meet the following requirements:Completion of six postsecondary semester or equivalent quarter units in early childhood education or child development. (or enrollment) Based on interviews and record reviews, the licensee did not comply with the section cited above in 2 out of 7 staff (S2&3) which poses a possible health, safety or personal rights risk to persons in care.
