Report finding: Lead Testing (1) A licensed child day care center, as defined in Section 1596.76, that is located in a building that was constructed before January 1, 2010, shall have its drinking water tested for lead contamination levels on or after January 1, 2020, but no later than January 1, 2023, and every five years after the date of the initial test. This requirement is not met as evidenced by: Based on interview and record review, the licensee did not comply with the section cited above which poses a potential health, safety or personal rights risk to persons in care. Director was unable to provide the lead test result/documents.

This requirement is not met as evidenced by: Based on observation, the licensee did not comply with the section cited above which poses a potential health, safety or personal rights risk to persons in care. LPA observed one fence in the garden area is unstable.

Report finding: Responsibility for Providing Care and Supervision: a) The licensee shall provide care and supervision as necessary to meet the children's needs.(1)No child(ren) shall be left without the supervision of a teacher at any time...Supervision shall include visual observation. This requirement was not met as evidenced by: On 12/2/2025, Child #1 (C1) was found unsupervised in the outdoor playhouse for approximately 5–10 minutes. Staff S1 did not conduct a headcount during the transition from outdoor to indoor activities, resulting in C1 being left outside unattended.

(1) No child(ren) shall be left without the supervision of a teacher at any time, except as specified in Sections 101216.2(e)(1) and 101230(c)(1).Supervision shall include visual observation. This requirement was not met as evidenced by: On 1/30/2025 staff (S-1) was observed by staff (S-2) leaving a child unsupervised outside the door that leads to the playground. Staff (S-1) and the other children entered the classroom and the door was closed, leaving child (C-1) unspervised outside. Staff (S-2) immediately told staff (S-1) that child (C-1) was outside and redirected child back inside classroom. This poses an immediate threat to the health and safety of children in care.
Inspector notes: This requirement was not met as evidenced by: On 1/30/2025 staff (S-1) was observed by staff (S-2) leaving a child unsupervised outside the door that leads to the playground.
