Indoor Activity Space for Infants: (b) Indoor activity space for infants shall be physically separate from space used by children in the child care center and school-age child care center components. This requirement was not met as evidenced by: Based on interviews, records reviews and evidence gathered, during the month of March 2026, multiple infants(C1,C2,C3, C4 &C5) were moved to the preschool age Room 7 on mupltiple occassions. Director self-admitted that sometimes infants from Room 4 are moved to Preschool age Room 7 from 45 minutes to 2 hours unless infants are taking a nap in the Room 7. Director was advised that on multiple occassions, multiple infants have spend more than 2 hours in a Preschool age Room 7 in the month of March 2026, which poses a potential Health, Safety, or Personal Rights risk to persons in care.

Obtain a California clearance or a criminal record exemption as required by the Department This requirement was not met as evidenced by LPA observed S1 did not have fingerprint clearance and had been working at the facility since 06/09/2025. S1 stated she was informed around September that she was not cleared and her boss (Renee Torres) had been checking daily. LPA observed S1 was sent again to a live scan office on 10/01/2025 to obtain clearance. S1 stated she was informed she was not cleared recently and was sent on 03/06/2026 to the same live scan office to obtain clearance for the third time. This poses an immediate risk to the Health, Safety or Personal Rights to children in care.

This is an Amended report for visit conducted on 02/12/2026 There shall be a ratio of one teacher visually observing and supervising no more than 12 children in attendance. This requirement was not met as evidenced by LPA's review of attendance records, staff time cards and daily classroom logs. The records indicated that Room 7 had 16 children with S6 who arrived at 8:06am, S1 did not arrive until 12:06pm, Room 8 had 17 children with teacher S2, S5 did not work that day, Room 10 had 11 children with S4, Room 12B had 10 children with S3. This information was confirmed with the Director and Assistant Director, and both of whom acknowledged that the facility was out of ratio on that day due to low staffing. This poses an immediate risk to the Health, Safety or Personal Rights to children in care.
