This requirement is not met as evidenced by: Based on observation and interview, the licensee did not comply with the section cited above which poses a potential health, safety or personal rights risk to persons in care. Outdoor Activity Space: (d) The surface of the outdoor activity space shall be maintained: (2) Free of hazards including, but not limited to, holes, broken glass and other debris, and dry grasses that pose a fire hazard. This requirement is not met as evidenced by: Based on observation and interview, the licensee did not comply with the section cited above which poses a potential health, safety or personal rights risk to persons in care. There were multiple uneven (broken) concrete surfaces on the playground, presenting tripping hazard.
Responsibility for Providing Care and Supervision: (a) The licensee shall provide care and supervision .to meet the children's needs. (1) No child(ren) shall be left without the supervision of a teacher at any time, ... Supervision shall include visual observation. This requirement is not met as evidenced by: LPA Jimenez found that during a cutting activity, classroom cutting activity the children were using sharp metal scissors and while this cutting activity facility staff moved from their assigned areas without coordinating visual supervision for a lapse of seconds and did not witness the incident resulting in a day care child sustaining an injury and staff allowed children the use of sharp metal scissors. This poses a potential health and safety risk to children in care.
