(e) All individuals subject to a criminal record review... shall prior to working, residing or volunteering in a licensed facility: (1) Obtain a California clearance or a criminal record exemption as required by the Department. (2) Request a transfer of a criminal record clearance as specified in Section 101170(f). This requirement is not met as evidenced by: During today's inspection, LPA observed staff S1 showing an “In Process” background clearance. Director confirmed that S1 has been employed at the facility since 08/18/2025. No proof of criminal record transfer request or clearance before 08/18/2025 was provided. This poses a potential risk to the health, safety and personal rights of children in care.

This requirement is not met as evidenced by: Based on record review, LPA observed that child’s authorized representative was not informed of bump (insect bite) on the child that child received at the facility in a timely manner.
