Responsibility for Providing Care and Supervision. The licensee shall provide care and supervision as necessary to meet the children's needs. No child(ren) shall be left without the supervision of a teacher at any time.... Supervision shall include visual observation. This requirement is not met as evidenced by: By POC 04/24/2024, Director will submit plan on how she will ensure staff are supervising at all times. Based on interview and record review, S-1 fell asleep during naptime and was the only staff present in the room at the time. This poses an immediate health and safety risk to children in care. S-1 was terminated on 04/18/2024.

Reporting Requirements. Upon the occurrence, during the operation of the child care center of any of the events specified in (d)(1) below, a report shall be made to the Department by telephone or fax within the Department's next working day and during its normal business hours. In addition, a written report containing the information specified in (d)(2) below shall be submitted to the Department within seven days following the occurrence of such event. Any unusual incident or child absence that threatens the physical or emotional health or safety of any child. This requirement is not met as evidenced by: An incident occured on 5/17/23 where staff fell asleep while supervising children in the Young Toddler Classroom during naptime. No other staff were present.This Incident was not reported to Licensing, which poses a potential health and safety risk to children in care.
