Building and Grounds. The child care center shall be clean, safe, sanitary and in good repair at all times to ensure the safety and well-being of children, employees and visitors. This requirement is not met as evident by: Based on observation and record reviews, facility had a wooden beam that went across the pergola that was about 3 feet off the ground. LPA observed that children had to bend down or squat down to go under. There was an incident where a child bumped into it and obtained a red mark on their face, which poses a potential health and safety risk to children in care.
All individuals subject to a criminal record review pursuant to Health and Safety Code Section 1596.871 shall prior to working, residing or volunteering in a licensed facility: (1)Obtain a California clearance or a criminal record exemption as required by the Department or This is requirement is not met as evident by: Based on record review, S-2 completed livescan on 06/29/2022; however, facility number was entered incorrectly. S-2's fingerprints are not cleared and associated to facility, which poses an immediate health and safety risk to children in care.
Reporting Requirements. Upon the occurrence, during the operation of the child care center of any of the events specified in (d)(1) below, a report shall be made to the Department by telephone or fax within the Department's next working day and during its normal business hours... This requirement was not met as evident by: Based on interview, Site Director Jessica stated that she did not report one of the cases of COVID to Licensing within the next working day. This poses a potential risk to the health and safety to the children in care.
