- Teacher-Child Ratio. There shall be a ratio of one teacher visually observing and supervising no more than 12 children in attendance, except as specified in (b) and (c) below. This requirement is not being met due to although during nap time ratio is 1 teacher/aide to 24 napping children, when there is a child(ren) off of their mat during nap time they are no longer considered to be napping and ratio must then revert to 1 teacher to 12 children. This poses a potential risk to the health and safety to the children in care.

- Licensure Requirements Every licensed child day care center shall have one or more carbon monoxide detectors in the facility that meet the standards established in Chapter 8 (commencing with Section 13260) of Part 2 of Division 12. The department shall account for the presence of these detectors during inspections. This requirement is not met as evidenced by: Based on observation, the licensee did not comply with the section cited above in which poses a potential health, safety or personal rights risk to persons in care. LPA observed Facility did not have a carbon monoxide detector in any of the classroom. Director stated that director thought that facility had dual fire and carbon monoxide detector.

- Administration of Child Day Care Licensing (1) On or before March 30, 2018, a person who, on January 1, 2018, is a licensed child day care provider, administrator, or employee of a licensed child day care facility shall complete the mandated reporter training provided pursuant to paragraphs (2) and (3) of subdivision (a), and shall complete renewal mandated reporter training every two years following the date on which he or she completed the initial mandated reporter training. This requirement is not met as evidenced by: Based on observation and record review, the licensee did not comply with the section cited above in which poses a potential health, safety or personal rights risk to persons in care. LPA observed S1 did not have a current Mandated reporter training certificate, Director stated that staff forgot to do the mandated reporter training.
