The licensee shall provide the Department with a copy of an inspection report when an inspection is required by the local building inspector as a result of the alteration, addition or construction. This requirement was not met as evidenced by Licensee stated they did not obtain permits for the changes completed in the home. This poses a potential risk Health, Safety Personal Rights risk to children in care.
The home shall be free from defects or conditions which might endanger a child. This requirement was not met as evidenced by LPA observed live cockroaches in the facility. This poses a potential risk Health, Safety Personal Rights risk to children in care.
Immunizations (g) The licensee shall document each child's immunizations as required by the California Code of Regulations, Title 17, Section 6070, and shall maintain such documentation for as long as the child is enrolled. (1) This requirement includes updating each child's PM 286 (6/95) when the child is due to receive required immunizations after enrollment in the family day care home. This requirement is not met as evidenced by: Based on record review, the licensee did not comply with the section cited above. Child 3 and 6 immunization records need to be updated which poses/posed a potential health, safety or personal rights risk to persons in care.
