Lead Testing Written Directives (b)(1) a result with values of 5.5 ppb or greater shall be deemed an Action Level Exceedance. This requirement was not met as evidenced by: Six water fixtures reported an ALE of 5.5 parts parts per billion or greater. Fixture "B" = 5.5, Fixture "G" = 8.8, Fixture "I" = 8.5, Fixture J = 6.9, Fixture KK = 6.7, and Fixture "N" 6.9. This poses a potential risk to the health, safety, and personal rights of children in care.

Responsibility for Providing Care and Supervision: No child(ren) shall be left without the supervision of a teacher at any time, except as specified in Sections 101216.2(e)(1) and 101230(c)(1). Supervision shall include visual observation. This requirement was not met as evidenced by: one preschool child followed a parent that had propped open the door leading to the playground back inside the Facility on June 27, 2023. This presented a potential risk to the health/safety of the child in care.
