Buildings and Grounds – (a) the child care center shall be clean, safe, sanitary and in good repair at all times to ensure the safety and well-being of children, employees and visitors (1) the licensee shall take measures to keep the center free of flies, other insects, and rodents......... This requirement is not met as evidenced by, on 11/10/2025 rodent droppings were discovered by a teacher in Mercury and Viking classrooms. On 11/12/2025 rodent was discovered in Apollo and Vanguard classrooms by a teacher. This poses a potential risk to the health and safety of children in care.

This requirement is not being met as evidence by: LPA observed Dirt King (DK-250-R) tricyles on the play yard. Per manufacturer specifications, this particular model of tricyle is for ages 4-6. This play yard was for the younger preschoolers (2-3 year olds). This has the potential to be a health and safety risk to children in care.
