(4) Mattresses shall be made specifically for the size crib or play yard in which they are placed. This requirement is not met as evidenced by: Based on observation and interview, the licensee did not comply with the section cited above for two out of five play yards which poses a potential health, safety or personal rights risk to persons in care. Two out of three cribs were also obseved to be torn and LPA observed blankets covering the sides of some play yards and blankets inside some. No children were observed to be napping during the inspection.
