Responsibility for Providing Care and Supervision: (a) The licensee shall provide care and supervision as necessary to meet the children's needs. (1) No child(ren) shall be left without the supervision of a teacher at any time, except as specified in Sections 101216.2(e)(1) and 101230(c)(1). Supervision shall include visual observation. This requirement has not been met as evidenced by: Staff did not have 100% visual supervision of children in care which resulted in an unexplained injury for a child. This poses a potential health and safety risk to children in care.
Reporting Requirements: (b) The name of the child care center director, and any fully qualified teacher(s) designated to act in the child care center director's absence, shall be reported to the Department within 10 days of a change of child care center director or designee(s). This requirement has not been met as evidenced by: Facility did not submit the New Director's File to the Department within 10 days of the Director's resignation with the Facility on 02/12/2025. This poses a potential health and safety risk to children in care.
101700.3(b)(1) Testing results with fractional ppb readings of 0.5 ppb or greater shall be rounded up to the nearest whole number, before comparing to the Action Level.
