Reporting Requirements: Upon the occurrence, during the operation of the child care center of any of the events specified in (d)(1) below, a report shall be made to the Department by telephone or fax within the Department's next working day and during its normal business hours. In addition, a written report containing the information specified in (d)(2) below shall be submitted to the Department within seven days following the occurrence of such event. Events reported shall include the following: Epidemic outbreaks. This requirement was not met as evidenced by: Facility did not report the epidemic outbreak in December 2024 and May 2025, to the Department(Community Care Licensing) as required. This presents a potential risk to the health, safety, or personal rights of children in care.

Infant Care Personal Services: (b) The infant shall be kept clean and dry at all times. (2) Each infant's clothing and diapers shall be changed as often as necessary to ensure that the infant is clean and dry at all times. This requirement was not met as evidenced by: Per interview and record review, On 3/26/25, C1's diaper remained soiled for one hour and 25 minutes, which poses a potential risk to the Health, Safety or Personal Rights to children in care.
Personal Rights - (a) The licensee shall ensure that each child is accorded the following personal rights: (2) To be accorded safe, healthful and comfortable accommodations, furnishings and equipment to meet his/her needs. This requirement was not met as evidenced by: On 10/17/24, two children (C1 & C2) requiring milk substitutions were inadvertently served the incorrect milk. This posed an immediate risk to the health, safety, and personal rights to children in care.
