101223 Personal Rights (a) The licensee shall ensure that each child is accorded the following personal rights: (1) To be accorded dignity in his/her personal relationships with staff and other persons. (2) To be accorded safe, healthful and comfortable accommodations, furnishings and equipment to meet his/her needs. This requirement was not met as evidenced by: Based on the report and interviews with staff, the children's personal rights were violated on multiple occassions by staff (S1). This poses an immediate risk to the Health, Safety or Personal Rights to children in care.

Buildings and Grounds: The child care center shall be clean, safe, sanitary and in good repair at all times to ensure the safety and well-being of children, employees and visitors. This requirement is not met as evidenced by: Based on record review, the CCC Lead Sampling report for the facility indicated that water from the identified four (4) faucets have elevated lead above 5ppb which poses a potential health and safety or risk to persons in care.
