Supervision shall include visual observation. This requirement was not met as evidenced by: Based on interviews, and evidence gathered, Child (C1) was found outside of the play area alone without any supervision. This poses an potential risk to the health, safety and personal rights of children in care.

- (a) The child care center shall be clean, safe, sanitary and in good repair at all times to ensure the safety and well-being of children, employees and visitors. This requirement was not met as evidenced by : The director will made the A/C unit inaccessible to children and send LPA a picture of the completed project. Based on observation, the licensee did not comply with the section cited above. The A/C unit was exposed in the patio and accessible to children, which posed a potential health, safety or personal rights risk to persons in care. The facility refused to sign.
