Responsibility for Providing Care and Supervision: The licensee shall provide care and supervision as necessary to meet the children's needs.(1)No child(ren) shall be left without the supervision of a teacher at any time. Supervision shall include visual observation. This requirement was not met as evidenced by: Based on interviews, and evidence gathered, Child (C1) was found outside of the play area alone without any supervision. This poses an potential risk to the health, safety and personal rights of children in care.
(a) The child care center shall be clean, safe, sanitary and in good repair at all times to ensure the safety and well-being of children, employees and visitors. This requirement was not met as evidenced by : The director will made the A/C unit inaccessible to children and send LPA a picture of the completed project. Based on observation, the licensee did not comply with the section cited above. The A/C unit was exposed in the patio and accessible to children, which posed a potential health, safety or personal rights risk to persons in care. The facility refused to sign.
Administration of Child Day Care Licensing (1) On or before March 30, 2018, a person who, on January 1, 2018, is a licensed child day care provider, administrator, or employee of a licensed child day care facility shall complete the mandated reporter training provided pursuant to paragraphs (2) and (3) of subdivision (a), and shall complete renewal mandated reporter training every two years following the date on which he or she completed the initial mandated reporter training. This requirement is not met as evidenced by: Based on record review, the licensee did not comply with the section cited above, Mandated reporter training certificates were not observed for S3 and S4, which poses/posed a potential health, safety or personal rights risk to persons in care.
