(1) No child(ren) shall be left without the supervision of a teacher at any time... This requirement was not met as evidenced by: Based on interviews conducted and observations of video footage, two preschool children (C1 and C2) were left unsupervised in classroom 215 and unsupervised in the the school courtyard which poses an immediate risk to the health, safety, and personal rights of children in care.
