Administration of Child Day Care Licensing (1) On or before March 30, 2018, a person who, on January 1, 2018, is a licensed child day care provider, administrator, or employee of a licensed child day care facility shall complete the mandated reporter training provided pursuant to paragraphs (2) and (3) of subdivision (a), and shall complete renewal mandated reporter training every two years following the date on which he or she completed the initial mandated reporter training. This requirement is not met as evidenced by: Based on the document review, the licensee did not comply with the section cited above , the assistant's Mandated reporter training certificate has expired on 11/9/2025 which poses/posed a potential health, safety or personal rights risk to persons in care.
(a) Prior to making alterations or additions to a family child care home or grounds, the licensee shall notify the Department of the proposed changed, including, but not limited to, the following: (6) Any change from an area of the family child care home previously identified as "off limits" to an area where care and supervision will be provided to children in care. This requirement is not met as evidenced by: LPA observed the garage was converted into an ADU and a second ADU behind the garage, which the licensee stated are un-permitted. which poses/posed a potential health, safety or personal rights risk to persons in care.
